A selection of suitable home will be identified following a full assessment of your needs, and a report provided. Customised Care will take you or your representative to view up to three of the chosen facilities. |

A selection of suitable home will be identified following a full assessment of your needs, and a report provided. Customised Care will take your or your representative to view up to three of the chosen facilities. Customised Care will negotiate the fees on your behalf, and arrange an admission date. We will manage the move, liase with the home to set up a mutually acceptable plan of care, and act as a mediator and negotiator on your behalf. Customised care will then visit you at intervals following admission to the home. |
